  60. As noted in the catalog, you may also order via our toll-free 800 telephone
  61. number. The 800 number to use is printed on your membership card.

  65. (1) You must be an APDA member to order products. After submitting your
  66.     application with means of payment, you will receive your membership card
  67.     and order number by mail. The APDA membership application form is available
  68.     on AppleLink as a MacWrite file for you to down-load, fill in, and mail
  69.     if you are not already a member.

  71. (2) P.O. Boxes may NOT be used for U.P.S. shipments, please give a street
  72.     address.

  74. (3) Purchase orders are accepted only from educational institutions or
  75.     companies which already have a standard credit application on file.
  76.     If you order by specifying a purchase order # we will consider it to be
  77.     a 'verbal P.O.' and will hold the order until we receive the original,
  78.     signed P.O. by mail.

  80. (4) You will receive an "APDAlog" with your membership. Use the product codes,
  81.     descriptions, shipping weights and prices shown in the catalog for ordering
  82.     information. "APDAlog" details a shipping table. Calculate the shipping
  83.     charges based on the sum of individual shipping weights, and include
  84.     payment with order.
